A new plate, and the only one whose subject is time: five decades of statute, guidance, payment, measurement and models drawn as load rather than sequence—each instrument arrives at its date and keeps running while it remains in force, over a counted load line. Twenty instruments; seventeen still bearing weight; the two model terminations of 31 December 2025 are the only red on the plate. It sits under the second question, beside The Crosswalk.

Three entities leave the model—the mid-size organizations, the independents, the home programs—because they were segments of the certified-facility population drawn as if they were separate actors; their facts fold into the parent. A new relationship kind, operates, draws what was always true: the two large chains run most of the certified facilities. And the two payment side doors, TDAPA and TPNIES, become waypoints the money visibly routes through. The model now counts 45 entities and 78 relationships; three entities remain deliberately unwired—named instruments of other lines, drawn hollow, counted, never hidden.

The V-tag plate stops being a picture of a mapping and becomes the instrument you look a tag up in: open a Condition and its block resolves into individual tags; open a tag and the regulation and the interpretive guidance print verbatim beside the plate, with CMS’s own tag-to-tag cross-references drawn as arcs. Four tags are readable today; every other tick opens and says plainly that its text is not yet transcribed. The mapping is CMS’s; the reading is what this plate adds.

All seventeen condition ranges on The Crosswalk were checked against Appendix H itself, now held in the workshop. Sixteen stand exactly as drawn. One was stale: Physical environment (494.60) runs to V424, not V420—four tags added by the fire-safety rulemaking after the 2008 guidance this plate was first transcribed from. The plate now draws V400–V424. The tag census is also corrected: Appendix H opens 377 tag blocks, and the two figures this site briefly carried—380, then 363—were both counting errors, recorded here because an atlas that grades its sources has to grade itself the same way.

The 2026-07-10 entry below said the Conditions-to-V-tag join is one “no public resource draws.” That was wrong, and an expert reader would have known it: CMS publishes the join itself, as a three-column table in the State Operations Manual, Appendix H. The claim is corrected to what is actually true—this plate draws that join as one connected view instead of a 420-page table. Appendix H now sits in the workshop as a primary source.

Nine relationships join the dataset: the four remaining ESRD Network contractors to CMS, the measure-development contractor to the data it computes from and the program it scores, and the Inspector General to its department. The published gap falls from sixteen entities to ten. The ten that remain are still drawn hollow and still counted—five of them wait on a question the model has not answered yet, and five need a kind of line the model cannot yet draw.

The entry becomes the instrument: the atlas itself—every entity and every relationship—drawn as a radial dial. Hover isolates an entity’s true edges; every count on the page is computed from the data at load; the model’s own gaps are drawn hollow and counted, never hidden.

The wordmark drops to lowercase wherever it navigates, the landing opens on the filed question, and the site signs itself: written, drawn, and kept by one author.

The plates now read in the order of the system itself: the person (The Crossroads, The Border, The Transplant Map), then the data the treatment produces (The Draws, The Data Map, The Stakeholder Matrix), then the machinery that judges it (The Benchmark Pipeline, The Two Ladders, The Crosswalk), then the money and the calendar (The Coverage Map, The Money Flow, The Regulatory Year-Wheel). The Patient Journey retires—every route it drew now lives on a stronger plate. Entries below this line cite the numbering of their own date.

This changelog, the references, and the reader’s shelf open. Source-currency dates extended to every plate—each now says when its sources were last verified, or the date of the dataset it was drawn from.

Four plates join the atlas: The Crosswalk—the join between the Conditions for Coverage and their V-tags, drawn as one connected view instead of a 420-page table—The Money Flow, The Two Ladders, and The Transplant Map. The Time Decoder, three clocks over one span of care, joins Plate III. Last-verified ledgers debut on the new plates.

The author page states its independence plainly and adds a limits line: personal work, views the author’s own, not clinical or compliance advice. The Terrain learns to settle—every condition on the frontispiece is now a live link to its section of the law.

The essay claimed “383 V-tags.” Wrong: V383 is a tag number—the top of the reuse block—not a count, and CMS publishes no total. Corrected to the verified span, V100 to V773. This is exactly the kind of error the atlas exists to catch, so it is recorded here rather than papered over.

The moving frontispiece ships: 42 CFR Part 494 drawn as terrain, elevation from the density of the text itself. Four drawn maps join—The Crossroads, The Data Map, The Coverage Map, The Benchmark Pipeline. The plates renumber I–VII; an early plate retires. the-access.co goes live.

The essay rewritten and typeset—third state. The engraved portrait joins The Cartographer. Page margins simplified to what a print’s margin carries: plate, state, date.
